We accept most major insurances including: BCBS, HealthChoice, Aetna, Gore's Insurance, UHC, Medicare, Tricare, and Medicaid.
Copay is due at time of service. If your insurance is not listed we will do our best do become certified with your insurance company. We will be happy to see you at an out of network rate or you may use our private pay rates.
Private Pay Acute Visit: $60 (Not including procedures, labs, or injections)
Private Pay Yearly and 1st Visit Exam and History: $80 (Not including procedures, labs, or injections)
DOT Physicals: $85
School Physicals: $15
Low income pricing available with proof of income.
Virtual Visits: $40
Home Visits: Private Pay $99 within a 20 mile radius (Medicare and Medicaid patients may qualify for home visits.)
Botox and Fillers
Botox: $11 per unit
Weightloss $75
Fillers: Depends on what the patient desires- pricing varies and is $600+ and will be discussed at length before any procedures are completed.
